Essais cliniques sur la maladie de Stargardt

Des essais cliniques encore rares…mais de plus en plus nombreux

Les essais cliniques sur la maladie de Stargardt  portent sur différentes technologies : thérapie génique, thérapie cellulaire, pharmacologie. Certains sont en cours de réalisation.

Ces essais constituent de nouvelles étapes dans la recherche d’un traitement mais ne confirment pas aujourd’hui l’existence d’un traitement spécifique efficace.

Essai clinique : définition

On entend ici par essai clinique, une étude médicale qui a pour objectif de vérifier l’efficacité et la tolérance d’un traitement chez l’être humain. Ces études sont réalisées tout d’abord en laboratoire en recherche fondamentale, puis sont testés soit sur des modèles cellulaires soit chez l’animal en vue de faire des premiers tests de tolérance (étude de doses, d’innocuité, d’efficacité).

Ensuite , les essais cliniques peuvent être réalisés chez l’Homme.

Chaque essai clinique est conçu et réalisé en conformité aux règles éthiques établies par les institutions de santé. Chaque personne incluse dans un essai clinique reçoit un suivi médicale et biologique tout au long de l’essai selon un protocole précis.

Un point sur la recherche dans le monde :

Pour mettre à jour l’information, nous utilisons depuis plusieurs années le site américain www.clinicaltrial.gouv qui est, à notre sens, le plus complet des sites internet.

Comme vous le savez, c’est à l’issu des essais cliniques sur l’homme avec la phase I/II puis phase II/III pour les maladies rares, que des projets de traitements deviennent des médicaments approuvés par les agences règlementaires de la santé.

La recherche ne se limite pas cependant aux seules équipes qui ont atteint le stade de l’essai clinique. Beaucoup d’autres équipes académiques (équivalent INSERM, CNRS) et privées (sociétés de biotechnologie, laboratoires pharmaceutiques) sont très actifs sur la recherche de tel ou tel technologie pour aboutir à un traitement de la maladie de Stargardt. Ces équipes sont, elles, bien difficiles à identifier, notamment pour des questions de concurrence et confidentialité des projets.

Voici donc la longue liste par technologie, avec dans certains cas des commentaires en français de la Fondation. La liste des essais cliniques ci-dessous est longue et c’est tant mieux ! Stargardt concerne 6 000 français environ, elle demeure une maladie fréquente pour une maladie rare.

Bien sûr, que toute cette effervescence scientifique va aboutir à des résultats, difficile de dire quand les 1ers traitements arriveront, dans quelques mois, dans quelques années, gardons nos forces pour « encore » patienter !

* Phase I/IIa Study of SAR422459 in Patients With Stargardt’s Macular Degeneration. Completed. Phase I/II – Sponsor: Sanofi – Assay USA and France. NCT01367444. SAR422459 is a AAV vector, which encodes the ABCA4 gene.

Commentaire de la Fondation STARGARDT (traduction):  La phase I// de l’étude sur des patients Stargardt est terminée. Dirigée par SANOFI réalisée aux USA et en France. C’est un AAV (adeno associated virus) qui apporte le gêne ABCA4 « dans la rétine ».

 

* Phase I/​II Follow-up Study of SAR422459 in Patients With Stargardt’s Macular Degeneration, Sponsor: Sanofi – Assay USA and France. NCT01736592. SAR422459, a lentiviral vector which encodes the ABCA4 gene.

FS: A nouveau SANOFI qui utilise cette fois un lentivirus pour transporter le gène.

 

* Nanoscope Therapeutics Inc. announced in July 2022 that the first patient has been dosed in a Phase 2 trial of its Multi-Characteristic Opsin (MCO-010) ambient-light activatable optogenetic monotherapy for Stargardt disease.

Selon la société, des données de sécurité et d’efficacité sur 6 mois sont attendues au premier trimestre 2023. La thérapie génique MCO-010 reprogramme les cellules rétiniennes saines pour les rendre photosensibles

FS : Technologie très innovante, l’idée est d’utiliser des cellules de la rétine en bon état et de les transformer en photorécepteurs pour redonner de la vision à ceux qui n’en n’ont presque plus ! Cette technologie ne vise pas uniquement les Stargardt mais la plupart des maladies rétiniennes pour lesquelles les photorécepteurs sont impliquées.

FS: COAVE est une société de biotechnologie française qui utilise comme plus haut les doubles vecteurs pour transporter le lourd gène ABCA4. A notre connaissance le projet n’est pas encore en étude de toxicologie !

 

* Gene supplementation, Nanoparticles ABCA4. Self-assembled nanoparticles of ECO, a multifunctional pH-sensitive amino lipid, and an ABCA4 plasmid. University of Oklahoma Health Sciences Center, USA. Trials have yet to be initiated.

* Gene supplementation, Nanoparticles ABCA4, a hybrid nonviral system composed of a multifunctional lipid ECO and a G4 nanoglobule designed for efficient gene delivery.  Case Western Reserve University, USA. Trials have yet to be initiated.

* Gene modulation, anti-sense oligonucleotides (AON) for ABCA4. AON acts asRNA modulators and they bind to complementary nucleotides ofeither mRNA or pre-mRNA, preventing splicing at deep intronic premature splice sites or enforcing exon skipping to prevent toxic protein production. Astherna, The Netherlands, Trials have yet to be initiated.

FS : Cette société hollandaise utilise la technologie des anti sens nucléotide (ASO) pour neutraliser les effets délétères du gène ABCA4 muté.

 

* Gene editing, CRISPR for ABCA4. This method removes mutations and surrounding nucleotides and replaces them with corrected sequences. University Hospital Tübingen, Germany. Trials have yet to be initiated.

FS : Ici l’équipe de Tubingen en Allemagne utilise la technologie des ciseaux CRISPER/CAS9 pour tenter de corriger la mutation sur le gène ABCA4.

* Pharmacodynamic Study of Emixustat Hydrochloride in Subjects With Macular Atrophy Secondary to Stargardt Disease. Emixustat modulates the visual cycle by inhibiting a critical enzyme of this pathway, and decrease the accumulation of A2E and to protect the retina from light-induced damage. Phase II – Kubota Vision Inc. Completed. NCT03033108. Emixustat delayed dark adaptation at 5 and 10 mg doses, confirming the drug’s biological activity.

* Saffron Supplementation in Stargardt’s Disease. Phase I/II – Sponsor: Catholic University of the Sacred Heart – Assay, Italy. NCT01278277.

 

* Effect of DHA Supplements on Macular Function in Patients With Stargardt Macular Dystrophy and Stargardt-like Macular Dystrophy. Phase I/II – Sponsor: National Eye Institute (NEI). Assay in USA. NCT00060749. Data for 11 STGD1 patients reported no adverse events or signs of efficacy. Data for STGD3 cohort is yet to be published.

* Phase 2 Tolerability and Effects of ALK-001 (C20-D3-retinyl acetate) on Stargardt Disease (TEASE). Phase II – Sponsor: Alkeus Pharmaceuticals  Inc., Assay in USA. Recruiting. NCT02402660.

*Study of STG-001 (inhibitor of retinol-binding protein) in Subjects With Stargardt Disease. STG-001 is designed to reduce the uptake of vitamin A in the retina and potentially reduce the accumulation of retinal toxins for people with Stargardt disease and prevent retinal degeneration. Phase II – Sponsor: Stargazer Pharmaceuticals, Inc. USA. NCT04489511.

*Study to Evaluate the Safety and Efficacy of Tinlarebant in the Treatment of Stargardt Disease in Adolescent Subjects Lesion(s) in Adolescent Subjects With STGD1. Tinlarebant has been developed to slow the progression of STGD1 by reducing the amount of RBP4 in the blood and the supply of retinol into the eye. The reduction of retinol in the eye would reduce the cytotoxic lipids within the retina. Phase III, sponsor: Belite Bio, Inc, Taiwan. NCT05244304.

* Study of A1120 (inhibitor of retinol-binding protein) in Subjects with Stargardt Disease.  A1120-induced decrease in serum RBP4, partial depletion of certain visual cycle retinoids, and reduced accumulation of lipofuscin bisretinoids. Stargazer Pharmaceuticals, Inc. USA. Trials have yet to be initiated.

*Fenretinide is a synthetic derivative of vitamin A that binds retinol-binding protein. reVision Therapeutics, Trials have yet to be initiated.

*Therapeutic Potential of Omega-3 Fatty Acids Supplementation in Dry Macular Degeneration and Stargardt Disease (MADEOS). Prospective trial completed 2021. Ophthalmos Research and Education Institute, Cyprus. NCT03297515.

*Soraprazan (proton potassium-competitive acid-blocker developed for use in dyspepsia). It was granted orphan drug status by European Medicines Agency (EMA) for use in STGD in Germany in November 2013. Katairo GmbH, Germany. Trials have yet to be initiated.

*Isotretinoin delays visual loss in STGD but has adverse effects, including liver toxicity. The evidence comes from the mouse model (ABCA4–/–), in which it reduced A2E and lipofuscin granules but at a dose too high for human use. *Dobesilate was reported in a single case history to improve VA 4 weeks after a single injection, but with no changes in fundus photography, fluorescein angiography or foveal thickness. Dobesilate is an inhibitor of fibroblast growth factor. Hospital Universitario Ramón y Cajal, Spain.

Sponsored by the Foundation Fighting Blindness, the Natural History of the Progression of Atrophy Secondary to Stargardt Disease (ProgStar) studies are comprised of retrospective and prospective longitudinal observational studies. The retrospective study includes clinical examination findings and images collected between 2008 (and earlier) and 2014 and progression will be retrospectively evaluated. The prospective study consists of a 24 month observational period from 2013/2014 for two years, with one visit every six months.

 

ProgStar Study : A natural History of the progression of Stargardt Disease : retrospective and prospective Studies (Learn more >  Clinicaltrials.gov).  » With the ultimate goal of determining the best outcome measures for clinical trials, the ProgStar study currently enrolls patients with Stargardt disease type 1 (STGD1) caused by mutations in the ABCA4 gene. »
